The Welsh healthcare frameworks first came into operation in July 2006.
Bam and Interserve have renewed their places, but Laing O’Rourke has failed to repeat its success five years ago to be nudged out by Kier Regional.
But the contractor may still come up trumps on the major project healthcare framework, which is being introduced for the first time and has yet to be announced.
Under the refreshed framework arrangement, all NHS building and refurbishment jobs valued between £4m and £10m in Wales will have to be placed with the trio of contractors.
The framework has been divided into south east, south west and north wales regions and all three contractors have beaten rival bidders to take a place on each of the lots.
Despite heavy spending cuts, the NHS forecasts it will spend £90m to £300m through the Design for Life: Building for Wales framework arrangement over the next four years.
